Requesting Medical Records

You may request your medical records online, or complete an authorization form and email, fax or mail it to the addresses below. The authorization form must be completed in its entirety. If a question does not apply, please indicate so by placing โ€œN/Aโ€ in the space provided. Any form not completed in its entirety will be returned to its sender.

Business hours are 8:30 a.m. to 5 p.m., Monday through Friday, excluding holidays. 

Weather Emergency

In case of a weather emergency, the hospital will conduct business as usual. However, during severe weather, such as blizzards and hurricanes, we may ask you to encourage family and friends to stay home. We do not wish anyone to risk their own safety in order to visit the facility.

Room Assignments

Rooms are assigned depending on availability, patientโ€™s medical needs, and the patient population. Private rooms may be requested, but they may not be available due to medical necessity. Because of changes in hospital census, occasionally patient rooms may need to be changed.

 

Fire Safety and Drills

At Moss-Magee, we take fire safety seriously. That is why our staff receives ongoing training in fire safety and in responding to other emergency situations. For your safety, the hospital regularly conducts fire and emergency preparedness drills. You will be notified if a drill occurs during your hospital stay. When a fire emergency has been reported, strobes will flash, and doors will close automatically on the floor where a fire emergency has been reported, including the doors of patient rooms. The emergency response team staff will evacuate you, and other patients and visitors, to a safe area.

Nurse Call Button

If you need to talk to your nurse at any time, press the call button located by your bedside. If you are unable to press the call button, other devices are available. For your safety, there is also an emergency call button in the bathroom.

Pet Visitation

Certain pets are allowed as visitors. If you would like to have a pet visit, please discuss this with your case manager and provide veterinary records. Once records have been approved, the case manager will provide the guidelines for pet visits. Pets may only visit with patients in designated areas of the hospital including the 2nd floor and the 6th floor. Pets may only visit in patient rooms if patient is in a private room.

Staff and Patient Areas

Areas in the hospital that are designated as Authorized Personnel Only are restricted to Moss-Magee staff. For safety reasons, inpatients are not permitted to leave the building or enter the parking garage, except with hospital staff. Outdoor areas are available to patients and visitors on the 2nd and 6th floors.

Tobacco-Free

In the interest of your health and safety, and the health of others, please observe our tobacco-free policy. If you currently smoke, your inpatient stay is an excellent time to stop smoking. Patients at Moss-Magee may participate in a free smoking cessation program. Please discuss this with your case manager and physician.

Moss-Magee is a tobacco and smoke-free institution. Smoking and the use of tobacco products are prohibited throughout all of Moss-Mageeโ€™s buildings and on all hospital property.This includes the use of electronic cigarettes and chewing tobacco.Smoking is not permitted in any structure, including restrooms, offices, cafeteria, lounges, balconies, garage, parking lots, roof, in company vehicles or in personal vehicles when on the Moss-Magee campus. These guidelines are made known to all employees, patients and visitors.

Drugs and Alcohol

Recreational drugs and alcohol are absolutely prohibited. Use of these substances by patients or visitors will result in discharge from Moss-Magee.
